The investigation revealed that operation staff attempted to prevent a parent from leaving with their children in the absence of a court order.

Based on video evidence provided, a caregiver did not demonstrate good judgement when she recorded children when she was supposed to be supervising them and recorded them without the consent of the parents or guardians.

Management failed to deliver form 2866 child interview letter to the parents of two children interviewed by Child Care Regulation.

Based on information gathered, a child was able to exit their classroom to the playground unsupervised on two occasions. Also, video evidence provided shows a caregiver videoing children and failing to intervene while children are seen climbing furniture.

At time of inspection operation was observed with debrins in the classroom cabinets and diaper changing tables.

During inspection, the trashcan in the school-aged classroom did not have a lid on it and was used to dispose of food. Corrected at inspection.

During the inspection, the diaper changing station was noted to have debris present. During the inspection, it was noted that the classroom floors contained debris.

At time of inspection, two staff files did not have verification of completing operation orientation.

At time of inspection, two infants, seven and eight months were observed laying flat on the rug while drinking formula.

At time of inspection, cots were observed to not be labeled/assigned to children.

Operation failed to provide a written incident report regarding a situation that placed children at risk to the parents of the children involved in the incident.

During an inspection conducted as part of a DFPS investigation, the school age classroom was observed to be chaotic and out of control. Caregivers failed to intervene effectively to ensure children were behaving in a safe manner.

During a DFPS investigation it was determined that the center director demonstrated poor judgment when she interviewed a child involved in an incident in front of the parents of other children involved in the incident without the permission of that child?s parent. Further, she allowed those parents to ask the child questions about the incident and what led up to it. The parents who were present could have presented a risk to the child due to the highly emotionally charged atmosphere surrounding the incident, as well as the director failing to protect the child?s individual privacy.

During the course of a DFPS investigation, the operation was unable to provide an enrollment packet/required file for a child subject to the investigation.

During the course of a DFPS investigation it was determined that a caregiver failed to adequately supervise children in her care resulting in an incident involving children engaging in inappropriate behavior.

A caregiver did not demonstrate good judgment when giving two children ice coffee.

The after-school caregiver stated there were 13 children on the playground before we verified there were 14 in care.

The teacher in the PreK classroom stated there were 20 children in care when we verified to be 21 children in care.

Based on information gathered during a DFPS investigation, a caregiver was observed grabbing a child by the back of the shirt near their neck, the caregiver further admitted to doing so while trying to correct the child's behavior.

The operation failed to notify the parent of the allegations that involved a caregiver grabbing their child by the back of the shirt near the neck as a form of discipline. The parent was not notified of the allegations until the CCI Inspector made contact with that parent that their child was subject of an investigation regarding possible allegations.

In the Prek4 and Toddler classroom two radio extension cords were dangling which were accessible to the children. NOTE: CORRECTED AT INSPECTION

After a diaper change in the infant classroom the child wasn't taken to wash hands. In the Prek4 classroom after the teacher assisted a child with a change of clothes after having an accident the teacher nor child washed hands. NOTE: CORRECTED AT INSPECTION.

Based on information gathered, and interviews conducted it was determined the caregiver in the school-age classroom grabbed a child from around the waist lifting the child to the front door.

At time of inspection, an infant teacher did not wash childs hands after diaper changing. This was corrected at inspection.

An eight-month-old infant was observed sleeping in the crib with the pacifier in their mouth with a pacifier clip attached to the clothing. NOTE: This was corrected at inspection when the pacifier clip was removed.

In the infant classroom a four-month-old was observed in the same classroom with a 23-month-old. This is a 19-month age difference.

Based on the information gathered, observation and interviews conducted the children are not being offered an alternative quiet activity while waiting on their cots while others are sleeping. NOTE: This was corrected after another walkthrough was conducted as children had books.

A caregiver was observed allowing infants aged 14-22 months view a tablet with a screen time activity. The screen time activity was while children were eating their lunch. NOTE; This was corrected during inspection as the caregiver turned off the program when she was informed that screen time was prohitted for children under the age of 24 months.

During the investigation, it was determined that no incident reports were completed or provided to parents regarding the incident.

During the investigation it was determined that a group of children were left unsupervised in a classroom for somewhere between five to ten minutes when caregivers failed to communicate to one another their intention to leave the classroom briefly.

During this investigation, it was determined that parents were not notified of incident that may have placed children at risk while at the operation.

One of the four children with diagnosed food allergies didn't have an Emergency Action Plan available in child's file. In addition to an Allergy and Dietary Preference List posted. One of the ten children didn't have Emergency Authorization information available for review. NOTE: Emergency Authorization information was corrected at inspection when provided electronically.

Five of the ten children files did not have updated immunization records. NOTE: One of the five immunization records was updated at inspection.

On 3/17/2023 there were 25 children in the care of one caregiver with a specified age group of four.

A toilet was observed to have excrement on the seat. This was corrected at inspection when the caregiver cleaned and sanitized the seat.

In one room children were not sleeping on their assigned cots and an assignment map was not available in another room.

After having their diaper changed a child's hands were not washed. This was corrected at inspection when the caregiver washed the child's hands and the hands of another child who had their diaper changed.

One of the caregivers involved did not have current CPR and First Aid training.

The sink identified for hand washing only was being used for washing infants cups, spoons, and plates. The sink was cleared, infants plates, bottles, and plates were removed.

Minibus #10277 child alarm system was tested and did not activate during the inspection.

A 16 month old infant was observed walking around while drinking a bottle. Note: This was corrected at inspection as the caregiver seated the child in a highchair.

A caregiver did not interact in a positive manner when they forcefully grabbed and picked up a child during redirection.

The fire inspection was expired at the time of inspection.

The incident report was not signed by the parent within 48 hours of incident where a child was injured and required medical treatment.

Based on the information gathered the area was not disinfected and sanitized when a child was injured and required medical treatment.

Based on the information gathered, the two caregivers were tending to one child on the playground to get the child ready to go home when a child fell off the playground structure and was injured. Both caregivers, had their backs turned and did not see the child fall.

A caregiver left children unsupervised on the playground.

Caregivers in the infant room did not know ages of the children in their care.
